 Eligible
participants must live in the plan’s service area
(Cattaraugus County or
a resident of the following Allegany County townships:
Amity, Angelica, Belfast, Bolivar, Clarksville, Cuba,
Genesee, Friendship, New Hudson and Wirt); be 55 years old or older;
determined eligible for nursing home level of care; in
need of the long term care services of the plan for 120
days; able to live safely in the community with the
services of the plan; have either Medicaid or Medicare
or be willing to pay privately.
**Participants may be
liable for payment of services not authorized by the
plan.
